Heat Illness in Athletes: How to Recognize Heat Cramps, Heat Exhaustion, Heat Stroke and Rhabdo
Colorado athletes are used to training hard. But when summer temperatures climb across Denver and the Front Range, normal practices, camps, conditioning sessions and outdoor workouts can become much more demanding on the body.
Football players are putting pads back on. Soccer and cross-country athletes are logging miles. Volleyball camps and preseason conditioning are underway. At the same time, construction crews, landscapers and other outdoor workers may spend hours performing physically demanding work in the heat.
That makes knowing the signs of heat-related illness especially important.
Heat illness isn't simply being hot, thirsty or tired. When the body can no longer manage the combination of environmental heat and physical exertion, the situation can become serious quickly.
Here are four conditions athletes, parents, coaches and outdoor workers should know.
Heat Cramps: An Early Warning to Stop and Recover
Heat cramps are painful muscle cramps or spasms that can occur during or after strenuous activity in hot conditions. They often affect the legs, arms or abdomen.
Heavy sweating and the resulting loss of fluids and salt can contribute.
Common signs of heat cramps include:
Painful muscle spasms or cramping
Heavy sweating during activity
Cramping during or shortly after exercise
If cramps begin, stop the activity and move to a cooler location. Rest, cool down and replace fluids. A sports drink containing electrolytes may also be appropriate after prolonged, heavy sweating.
Don't treat a significant heat cramp as something an athlete simply needs to “push through.” It is a reason to stop and assess how the athlete is handling the conditions.
The CDC recommends waiting for cramps to resolve before returning to physical activity.
Heat Exhaustion: Know When the Body Is Struggling
Heat exhaustion is more serious.
It can occur when the body is losing significant water and salt through sweating and struggling to keep up with the demands of activity and environmental heat.
Signs of heat exhaustion can include:
Heavy sweating
Cold, pale or clammy skin
Dizziness
Headache
Nausea or vomiting
Weakness or unusual fatigue
Muscle cramps
Fast, weak pulse
Fainting
If you suspect heat exhaustion, stop the activity immediately.
Move the person to a cool or shaded area, loosen unnecessary clothing or equipment, begin cooling them with cool wet towels or water, and have them sip cool water if they are alert and able to drink safely.
Seek medical attention if symptoms worsen, vomiting occurs, or symptoms do not resolve.
Heat Exhaustion vs. Heat Stroke: What's the Difference?
This is one of the most important distinctions for parents and coaches to understand.
With heat exhaustion, an athlete may be dizzy, nauseated, weak and sweating heavily, but they typically remain mentally coherent.
With exertional heat stroke, changes in brain function are a major warning sign.
Watch for:
Confusion
Disorientation
Slurred speech
Unusual behavior
Loss of coordination
Stumbling
Collapse
Loss of consciousness
An athlete with heat stroke may still be sweating. Do not assume someone can't have heat stroke simply because their skin isn't dry.
Suspected heat stroke is a medical emergency.
Call 911 immediately and begin aggressive cooling.
Move the person out of direct heat, remove excess clothing or equipment and cool the body rapidly. For exertional heat stroke in athletic environments, cold-water immersion is considered the most effective cooling method when trained personnel and appropriate equipment are available.
Do not delay emergency action while waiting for every symptom to appear.
Rhabdomyolysis in Athletes: The Heat-Related Risk Many Families Don't Know
There is another serious condition athletes should know about: rhabdomyolysis, often shortened to rhabdo.
Rhabdo occurs when damaged muscle tissue breaks down and releases proteins and electrolytes into the bloodstream. In severe cases, this can damage organs, including the kidneys.
Strenuous physical exertion and exposure to heat can both increase risk.
It deserves particular attention when someone suddenly returns to very intense workouts, camps or conditioning after a period of lower activity.
Warning signs can include:
Muscle pain that is much more severe than expected
Significant muscle weakness or unusual fatigue
Muscle swelling
Dark, tea- or cola-colored urine
Symptoms may not appear immediately. They can develop hours or even days after the activity.
If rhabdo is suspected, seek medical care immediately. Symptoms alone cannot confirm the condition; healthcare providers use blood testing, including creatine kinase or CK levels, to diagnose it.
This is not something to “sleep off” after a hard practice.
Why the First Days of Hot-Weather Training Matter
One of the best ways to reduce heat-related risk is heat acclimatization: gradually allowing the body to adapt to exercising or working in hot conditions.
That adjustment does not happen after one practice.
With repeated exposure, the body becomes better at sweating efficiently, regulating circulation and performing work at a lower core temperature and heart rate. CDC/NIOSH guidance for workers describes acclimatization as a gradual process generally occurring over approximately 7–14 days.
The same principle matters in sports.
Going directly from a relatively quiet summer into full-intensity conditioning in high heat is very different from gradually increasing workload.
Coaches should consider temperature, practice intensity, equipment, conditioning level, rest opportunities and athlete acclimatization rather than treating every practice day the same.
How Athletes Can Reduce Their Risk in Extreme Heat
A few simple habits can make hot-weather training considerably safer.
Hydrate before you're thirsty. Don't wait until halfway through practice to begin thinking about fluids.
Build in real rest breaks. Shade and cooling periods aren't signs of weakness. They're part of managing training load in difficult environmental conditions.
Use the buddy system. Athletes don't always recognize changes in themselves. Teammates may notice that someone has become unusually quiet, uncoordinated or confused first.
Adjust practice times when possible. Earlier morning and evening sessions can reduce exposure to the hottest part of the day.
Increase intensity gradually. This is especially important at the beginning of a season, camp or return to training.
Pay attention to equipment. Football pads, helmets, work clothing and other gear can add to the body's heat burden.
Stop when something feels wrong. Feeling faint, weak, dizzy, confused or unusually ill is not something to “tough out.”
CDC specifically recommends that athletes pace themselves, increase activity gradually, drink more water than usual during hot weather, and monitor teammates for signs of heat illness.
Preparing Your Athlete for the Season Means More Than Conditioning
Heat safety has to come first, and no manual therapy, mobility program or recovery treatment replaces appropriate hydration, acclimatization, rest, cooling or medical attention.
But preseason is also a good time to address the other physical problems athletes may have been carrying into camp.
If an athlete is already dealing with recurring knee pain, a restricted shoulder, an old ankle issue, limited mobility or a movement problem, adding weeks of practices and games can make those issues harder to ignore.
